Last Days of Life Care Plan

Aim:

This course is initially one hour with additional sessions on symptom management totalling three to four hours over a period of time

The vision of the New Zealand Palliative Care Strategy (2001) is that all people who are dying and their family/whanau that could benefit from palliative care, have timely access to quality palliative care services that are culturally appropriate and are provided in a coordinated way

Mercy Hospice Auckland, in partnership with the Auckland District Health Board, has been implementing the Last Days of Life Care Plan across the district

This tool has evolved from the NZ implementation of the Liverpool Care Pathway to transfer the hospice model of care into other settings. It is a multi-professional documented process which provides an evidence-based framework for last days of life care
