HOSPICE CARE AT HOME
Hospice comes to you
The Mercy Hospice community team visit patients, their families, and carers, in their homes.
The team includes:
- a Palliative Care Nurse Practitioner and experienced registered nurses
- a physiotherapist,
- a lymphoedema therapist
- the Family Support Team including social workers, counsellors, cultural support workers and a spiritual
- Support from Mercy Hospice Palliative Medicine Specialists, (senior doctors)
- Trained volunteers also assist with practical support.
The community team provides a high standard of care for you and your family to help you adjust to the changes in your lifestyle caused by your illness.

How we care for you at home
The Community Team Service include:
- Home visits and support for patients living at home or in residential care
- Thorough assessments and development of a plan of care
- 24-hour phone access for advice
- Liaison with general practitioners and other health professionals involved in the patient’s care
- Advice and support for carers
